Integrated Physician Network Supports Non-Profit to Address Social Determinants of Health

Primary Partners, an integrated network owned and led by independent physicians who value quality healthcare, announces the formation of a non-profit focused on reducing barriers to care and addressing social determinants of health in Central Florida. The Alliance for Community Health is committed to helping all Floridians access the care they need and the ability to lead healthier lives.

Alliance for Community Health strives to create stronger communities through better health.

We envision vibrant communities where all individuals are engaged in their own health choices, and they have the knowledge and the resources to access to programs that can make them healthier.

Our programs in 2020-21 will be focused on enabling access to behavioral care and vaccines.

We have hired Jane Simon, formerly of Johns Hopkins All Children’s Hospital and Feeding Tampa Bay, to raise our profile, build our programs, and create the partnerships we need to help individuals and families.
