Your Voice Matters on Local Health Issues

Sanford  - Every three years, tax-exempt hospitals and county health departments in Florida conduct a Community Health Needs Assessment (CHNA) and develop Community Health Improvement Plans (CHIPs) to address specific opportunities for improved community health. The Central Florida Collaborative comprised of AdventHealth, Orlando Health, Aspire Health Partners, Community Health Centers, Orange Blossom Family Health, True Health, Osceola Community Health Services and the Florida Department of Health in Lake, Orange, Osceola and Seminole Counties, has launched the 2022 Community Health Needs Assessment (CHNA). 

 

The Central Florida Collaborative needs your help to improve the region’s health. Share your insight about community strengths, resources, service gaps, and ways to address needs by completing the 2022 survey.

 The first step of this collaborative process is to survey community stakeholders. Seminole County residents are encouraged to participate in the 2022 survey. Community input is an important component of the plan to address community concerns and improve quality of life. It is important for residents to take the time and let their voice be heard. These voices influence health priorities for the coming years.  

 The survey can be completed by visiting the following link https://www.surveymonkey.com/r/CFC_CHNA_2022  or QR code below. Paper copies of the survey are also available upon request.

Results of the survey provide the foundation of the Community Health Assessment. Once the assessment is complete, it will be used to create three-year Community Health Improvement Plans.

Residents, employers and community partners are encouraged to share the link and ask others to take the survey. The survey will close February 4, 2022.
