Patient Centered Care: Re-defining How Healthcare Interacts

As healthcare continues to transition to an integrated, interoperable system, there are a multitude of currently fragmented components throughout healthcare causing a negative effect. The fragmented components create redundancies (wasteful spending), operational variances (lack of standardization), and unnecessary interruptions in service delivery causing gaps in Patient Centered Care Objectives.

Healthcare is taking a closer look into the different components as we review both sides; Health Care (Patient Perspective) and Healthcare (IDN/HCO Perspective).

The traditional healthcare model has changed due to reimbursement, breakthroughs in technology, and insurance coverage. Healthcare (IDNs and health systems) have traditionally placed the majority of resources at the acute care (hospital) level, however, there has been an industry shift as 93-97 percent of total patient encounters have navigated to continuum of care and alternate site locations. As more and more patient encounters occur outside of the acute care setting, there is a higher demand placed on the different provider segments that support alternate site locations throughout the patient's continuum of care.

The paradigm change as total patient encounters continues to navigate to alternate site locations places the patient's continuum of care at the forefront of industry discussions. Regardless of healthcare industry and/or departmental segment, there is an inherent need to address all factors surrounding cost, quality, outcomes (CQO initiatives), patient safety initiatives, and location security objectives throughout the patient's continuum of care.

Re-defining how healthcare interacts: Addressing the "common denominators" of healthcare and providing a complimentary resource to any/all provider locations.

Healthcare is a heavily regulated industry. Regardless of healthcare industry segment; federal requirements, accreditation guidelines, patient safety initiatives, and location security objectives are consistent. Each healthcare organization and entity is required to perform the necessary due diligence, ensuring the different requirements, guidelines, initiatives and objectives are achieved. As each organization and entity independently tries to obtain and verify vital information, it leads to redundancies, operational variances, and gaps that negatively impact the overall performance of healthcare.

Business Relationship Management (BRM) plays a vital role throughout healthcare. The healthcare delivery system mutually relies on providers (healthcare organizations) and healthcare industry companies (3rd party organizations including business affiliates, business partners, outsourced contingent workforce, and vendors and supplier companies) to support the overall's patient centered care. Each entity, regardless of industry segment, cannot function without the other.

Healthcare organizations and healthcare industry companies need a simplified system for data exchange that connects the currently fragmented healthcare delivery system - a cutting edge, innovative technology, and business model that streamlines the flow of vital information between organizations needed for federal requirements, accreditation guidelines, patient safety initiatives, and location security objectives. This is done by addressing the "common denominators" throughout healthcare providing the necessary tools, information, and resources in a mutually beneficial environment.

Healthcare is navigating toward an integrated, interoperable "community based" system. Accessing data in a "community based, shared services" environment lowers costs while increasing compliance.

Healthcare is expensive: compliance, risk management, patient safety, and security shouldn't be.

Healthcare impacts everyone and everyone's loved ones. In order to truly achieve cost savings and cost containment there is a need to address all segments of the healthcare delivery system that collaboratively support the patient's continuum of care and lowers costs for all organizations.
